A provider is someone who takes care of a recipient under the IHSS Program
(In Home Supportive Services). See below all requirements to become an
IHSS provider.

Attend Provider Orientation with Community Service Solutions. Alpine, Mono County or Inyo County offices are available.

Bring to Provider Orientation:
-Form of ID (drivers license, passport, state ID, or tribal ID card)
-Social Security card or Birth certificate
Step 4

After completing the Provider Orientation, sign an IHSS Program Provider Enrollment Agreement (SOC 846)

A recipient may apply for the In-Home Supportive Services (IHSS) program if you are over 65 years of age, disabled (children as well) or blind. IHSS is another alternative to out-of-home care.
Types of services authorized can include: housecleaning, meal preparation, laundry, grocery shopping, personal care services (such as bowel and bladder care, bathing, grooming and paramedical services), accompaniment to medical appointments, and protective supervision for the mentally impaired.

Must reside in your own home or a private residence of your choosing.
* Please note that hospitals, long-term care facilities, and licensed community care centers do not meet the 'private residence' requirement. -
Must submit a completed Health Care Certification form
